Single Applicant/ Signatory:
Clients purchasing shares for the first time should sign one of the spaces provided for the transferee's signature (i.e. the dark portion). A corporate entity should also affix the corporate seal beside the signatory's signature. In addition to the signatures, the investor's full name and postal address should be boldly written
(in block letters) in the space provided for the transferees address (i.e. the dark portion).
Joint Applicant/Signatory
Each of the two signatories should sign one of the spaces provided for the transferee's signature (i.e. the dark portion). A corporate entity should also affix the corporate seal beside each of the signatories' signature. In addition to the signatures, the investor's full name and postal address should be boldly written (in block letters) in the space provided for the transferees address (i.e. the dark portion).

Sales/Lodgement at the CSCS
Single Applicant/ Signatory
Clients sending in Share certificates to FSDH Securities for lodgement at the CSCS or sale should sign one of the spaces provided for the transferor's signature (i.e. the white portion). A corporate entity should also affix the corporate seal beside the signatory's signature. Please note that the portion for the name and address of the investor should be left blank.
Joint Applicant/Signatory
Each of the two signatories should sign one of the spaces provided for the transferor's signature (i.e. the white portion). A corporate entity should also affix the corporate seal beside each of the signatories' signature. Please note that the portion for the name and address of the investor should be left blank
When printing the forms, please ensure that the "print to fit page" button is unchecked on the print dialogue box.
